You must complete this form to and pay the amount of $25.00 to confirm the date of your hall rental. You may check out a hall key the week of your hall rental from the Tribal Office. Funeral/Anniversary services for tribal members and their families will not be charged and Funerals services and scheduled Tribal meetings will take precedence.
Hall Rental: Please keep the hall and all equipment in the same condition you received it in. You are responsible for supplying anything that is not listed below (the tribe’s cookware and dishes are not available for use). Please clean all grease on the stove and burners after use and please keep the tables and chairs clean
Checks can be made payable to: Cahuilla Band of Indians
I understand that by signing below, I am fully responsible for repairs or damages to the any items in the tribal hall and that the amount to repair or replace the items damaged may be deducted from my State Revenue Sharing or Per Capita distribution. A payment schedule may be agreed upon.
